Health Insurance Requirement and Card
NU-Q requires all currently enrolled students to have a valid State of Qatar national health insurance card and to submit a front and back copy of the Qatar national health insurance card to the NU-Q Counseling and Wellness office (room 3088) on or before the first day of the second week of class for the fall semester. Failure to comply with the above requirements will result in a student record hold due to noncompliance to national health insurance policy.Complete the health card application
If you are preparing to attend NU-Q but currently live outside of Qatar and do not possess a Qatar identification card, complete the health card application. Once you have completed the application you may submit the signed document to the Counseling and Wellness Office located in the NU-Q Student Affairs suite on or before the first day of the second week of class for the fall semester. Student Affairs will assist students in obtaining the Qatar national health insurance card once documentation is received and a resident permit is obtained for the student.

- Complete numbers 4-9 with your information

If you are preparing to attend NU-Q and you possess a Qatar identification card, you are responsible for obtaining and renewing your own State of Qatar national health insurance card. If you do not have a national health insurance card, you can apply for it by completing the health card application. Once you have completed the application, you may submit the signed document and any other required documents to the nearest primary health care center. If you already have a national health insurance card, please make sure it is valid. You can check the validity and renew your card, if necessary, online by visiting http://portal.www.gov.qa/. Click on “More” under “For Citizens and Residents” and then choose “Health Card."

- Leave blank numbers 1-3
- Complete numbers 4-15 and 17, as appropriate
- Complete number 16, “ID Number” with your Qatar ID number.
- Sign your name by “Applicant’s Signature”
As a benefit of being a student at NU-Q and a resident of the State of Qatar, all students will be given the Qatar National Health coverage as well as required to purchase supplementary private insurance.
Submit a copy of your card to NU-Q
Whether you are a resident of Qatar or not, once you have your Qatar health insurance card, submit a front and back copy to the Counseling and Wellness office on or before the first day of the second week of class for the fall semester.
StudentCare Plus Plan insurance requirement
NU-Q requires all currently enrolled students to participate in the private comprehensive health insurance plan and to submit all required documents to the NU-Q Health and Wellness Office on or before the first day of the second week of class for the fall semester.
The policy, at no cost to the student, is called the StudentCare Plus Insurance Plan and includes access to medical, financial and travel benefits, as well as coverage for pre-existing medical conditions. Failure to comply with the requirements will result in a student record hold due to noncompliance to private health insurance policy.

Once you have completed and signed the application, attach a front/back copy of your Qatar national health insurance card and a front/back copy of any other insurance certificates (e.g. parent’s health insurance). Students coming to NU-Q from outside of Qatar will not have a Qatar national health insurance card to submit at the time of this application and it is appropriate for these students to submit just the National Health Card application along with the Application for Insurance NUQ StudentCare Plus Plan.
Submit these documents to the Counseling and Wellness Office located in the NUQ Student Affairs suite, of the Carnegie Mellon Building, room 3088 on or before the first day of the second week of class for the fall semester. You may also submit your documentation via email as an attachment to patricia.collins@northwestern.edu. Please be aware that email is not completely secure. Once you are enrolled in the StudentCare Plus Insurance Plan, all communication from StudentCare will come directly to you via email (set email filter settings to allow receipt of StudentCare emails) or regular mail in care of the NU-Q Department of Student Affairs.

Upon presentation of your valid StudentCare Insurance card at Al Ahli hospital, your expenses will be directly billed to Studentcare Plus Insurance, provided that the medical costs do not relate to any of the exclusions listed under Section 1 Medical Expenses or General Exclusions as per your policy wording.
Al Ahli Hospital (AAH) has issued a notice that it intends to discontinue psychiatric treatment direct billing for all insurance companies commencing 1 January 2015 (all other direct billing benefits are unaffected).
Consequently, students presenting for psychiatric treatment at AAH from 1 January 2015 onwards will be expected to pay AAH prior to any treatment being provided. Psychiatric benefit claims are generally lower cost treatments and will need to operate on reimbursement basis, so we recommend students complete claim forms after they have been seen and we can give priority to assessing those ourselves, so disruption is minimised.
